| At the National Patient Safety Agency (NPSA) we believe that tackling patient safety in the NHS collectively and in a systematic way can have a positive impact on the care and efficiency of NHS organisations. |

| The NPSA helps the NHS learn from things that go wrong and develops solutions to prevent harm in the future. We work with patients and staff locally and nationally to foster a culture where errors can be reported and lessons learnt, and we analyse information from staff and patients collated via our national reporting and learning system and other sources. |

| The NPSA is also working with NHS staff and organisations to promote an open and fair culture, and to encourage staff to inform their local organisations when things go wrong. Working in partnership, the NPSA will help the NHS to establish safety as its first priority. |
