Improved Partnerships between Health and Local Government

Judges will be looking for evidence of radically improved partnership between health and local government, so submissions could demonstrate the following elements:
  • Evidence of initiatives where health and Local Government have worked closely together to address community health
  • Evidence of joint work between health and Social Care to tackle Long Term Conditions, reablement, personalisation 
  • Evidence of joint working between health and local government to address the wider determinants of health 
  • Evidence where working in partnership between health and local government has enhanced the experience of patients or the public in their usage of services

All entrants should be able to demonstrate how they built partnership working and what activities they undertook to ensure Local Government and health partners were jointly engaged in the programme.
